Yeah, thats the form. if you're from north campus, fill it up, go to college, get it undersigned by your principal and then along with xerox copy of provisional marksheet, submit at window 17 of the VC area (9:30 - 12:30 and 2-3 on weekdays, not possible on weekends) - they will ask you to pay 500 bucks at cash counter. retain the cash receipt and come after 7 days to window 17.

And what about this section : Name of the university to which student wants to migrate ?
 If suppose a candidate is applying to two different universities , then would he have to get 2 migration certificates issued ?

It wont matter - write anyone that you will take admission in, when the actual document comes, it wont be mentioned where you are migrating. it will be like a no-objection thing from the university.

No, just 5K at the time of counseling. Rest of the fees u need to deposit when session starts..
